I pictured Sherlock regaining consciousness shortly after Lestrade has been calling John's name and staying still to listen to see what his reception might be. Certainly John had a lot he could be very angry about, but he also had been entertaining suspicions about improbable outcomes, so he was somewhat prepared for finding Sherlock, possibly dead, in a flat rather than under his tombstone.
